Partnership in Practice: Who We Work With
We’re proud to work alongside some of the most dedicated and skilled professionals in the region. Our key partners include:
Kirklees Council – From social work and safeguarding to commissioning and adult services, our partnership with the local authority is fundamental to how we deliver care.
Huddersfield and Calderdale NHS Foundation Trust – We collaborate closely with hospital teams to ensure smooth discharges, clinical oversight, and continuity of care.
Locala District Nurses – A vital community partner, especially for those requiring wound care, medication support, or chronic condition management.
Kirklees Carephones – Their 24/7 telecare service offers safety and reassurance, particularly for individuals who are vulnerable or live alone.
Occupational Therapy Teams – We liaise with OTs to make sure individuals have the right equipment, home adaptations, and risk assessments in place to promote independence.
West Yorkshire Fire and Rescue Service – Their Safe & Well checks and home fire safety visits are an important part of our approach to risk reduction and wellbeing.
West Yorkshire Police – When issues around safeguarding, vulnerability or community safety arise, we maintain open and responsive communication with local officers.
Yorkshire Ambulance Service – We work together to ensure timely, appropriate, and informed handovers in urgent or emergency situations.
Other Local Care Providers – Collaboration isn’t competition. We value strong relationships with other providers across Kirklees, sharing best practice and ensuring continuity of care.
